Youth Permission/Medical Release Form

Any youth who wants to participate in activities planned through the Youth Fellowship Program that will involve leaving the church premises needs to have a signed permission/medical release form on file. You can download the form here and give the signed form to Rev. Jack or one of the other youth leaders.
